If the enrollment fee will be paid with a credit/debit card in a name other than the registering participant, please enter the cardholder's name in the Comments box. Otherwise, we may not match the payment to the correct registrant.
As a Reminder: This is NOT an introductory course or a review course. Travel, lodging and ground transportation are NOT included in the enrollment fee.
