IF YOU ARE A MEMBER OF A GROUP OR A RETURNING PARTICIPANT DO NOT REGISTER USING THIS PAGE! Please contact Jerry W. Jones, MD (jwjmd@medicusofhouston.com) and you will be sent a link to the appropriate page in order to complete your registration and payment. If you are part of a group, you must include the name of your group in the email.

If the enrollment fee will be paid with a credit/debit card in a name other than the registering participant, please enter the cardholder's name in the Comments box. Otherwise, we may not match the payment to the correct registrant.

PLEASE NOTE! These are NOT introductory or review courses. Travel, meals, lodging and ground transportation are NOT included in the enrollment fee.
